Welcome to the Student Health Services page! If you need access to any school-related medical forms and/or action plans for your child, this is the place to be. 

If your child suffers from asthma, diabetes, seizure disorder, food allergy and/or an insect allergy, you must complete an Action Plan. Your family physician should also sign the completed form. Then return it to your child’s school ASAP. 

If your child needs to take prescription medication or over the counter medication while at school, a Medication Authorization must be completed by the prescribing physician. Medication cannot be administered without the medication authorization form on file.

Also, the State of Illinois requires certain health requirements for students new to the district and for those at the grade levels listed below. With the exception of dental exams, all health records must be submitted by October 15th. Dental exams are due on May 15th

Please note that sports physicals do NOT count as a school physical. Also, if medical requirements are not met by the October 15th deadline, a child may be excluded from attending until the appropriate medical information is submitted. 

  • Preschool: School physical (a sports physical does NOT meet requirement), up-to-date immunization record, and lead screen

  • Kindergarten: School physical (a sports physical does NOT meet requirement), up-to-date immunization record, lead screen, eye exam (completed by an optometrist/ophthalmologist), and dental exam

  • Second Grade: Dental exam

  • Sixth Grade: School physical (a sports physical does NOT meet requirement), up-to-date immunization record, and dental exam

  • Ninth Grade: School physical (a sports physical does NOT meet requirement) and dental exam

  • Twelfth Grade: 2nd dose of meningitis vaccine

Any out-of-state students attending Illinois schools for the first time must have an Illinois physical exam, an eye exam, and proof of up-to-date immunizations prior to enrolling. If you do not provide documentation at the time of enrollment, you have 30 days to meet this requirement after registering your child

ou can call the Effingham County Health Department to make an appointment for immunizations.
 
If you would like to discuss health concerns for your child, please contact your child’s school secretary or principal and give a brief description of your concern. The district’s nurse works at every school, and the principals and secretaries all know how to reach her.
